AJAPRIDE-PM 1 is a widely prescribed triple-combination antidiabetic medication manufactured by Ajantha. Composed of 1mg of Glimepiride (a sulfonylurea), 500mg of Metformin Hydrochloride Sustained-Release (SR), and 15mg of Pioglitazone Hydrochloride (a thiazolidinedione), it is indicated for glycemic control in adult patients diagnosed with Type 2 Diabetes Mellitus when double combination therapy is insufficient.

Mechanism of Action

AJAPRIDE-PM 1 works through three distinct mechanisms:

  • Glimepiride: Binds to ATP-sensitive potassium channels on pancreatic beta-cells, causing depolarization and opening of calcium channels, triggering the release of insulin.
  • Metformin: Decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity.
  • Pioglitazone: Activates peroxisome proliferator-activated receptor-gamma (PPAR-gamma), regulating genes involved in glucose control and lipogenesis, increasing peripheral insulin sensitivity.

Safety Warnings

  • Lactic Acidosis: Metformin carries a rare but serious risk of lactic acidosis. Avoid in severe renal impairment.
  • Fluid Retention: Pioglitazone can cause fluid retention, which may lead to or exacerbate congestive heart failure. Contraindicated in NYHA Class III/IV heart failure.
  • Pregnancy & Nursing: Strictly contraindicated. Avoid use.
